Diabetic Mobile CareKit

SUMMARY

BUSINESS PROBLEM

Payors have Care Management platforms get data from multiple sources. Data is siphoned into it in various ways. However, these lack an out of the box integration with digital omnichannel sources like mobile phones and smartwatches. This leaves health plans/payers with a disadvantage because, in today's digital economy, insured members are consumers. Like any other consumers, insured patients generate all manners of data via digital means when engaged via the same platform. Not being able to integrate with digital devices, meant that the health plans now have to spend many resources in setting up an integration jungle to bring consumerism into the forefront.

SOLUTION

Under my direction, the UX team set out to come up with concepts on what will a mobile-based offering look like and what problem it would solve. After conducting a design sprint, we zeroed in on the need for health plans to engage their pre-diabetic members as that population was identified as the riskiest to become a burden. We designed and built the app as a platform, that would initially cater to all ranges of Diabetic members. The app would be interoperable with health care based IoT devices starting with the most prevalent fitness devices.

Using this mobile channel would not only allow health plans to do outreach with better outcomes but also expand their strategy on digital transformation.

MY ROLE

I envisioned the product and superrvised and contributed to the design. I provided all the journey flows for this concept. After that, I maintained a supervisory role to have the first set of Hi-Fi design built out in 2016. During 2019, I supervised the rework of this per the new HealthEdge Mobile Design system.

Flow

While I am not at the liberty of sharing complete flows at this time, the following screens capture the main use cases. A member first signs up and receives a care kit. Then the member can setup various aspects of the app and use both manual data entry, and IoT filled information.
